git credential manager: update links

The Git Fundamentals team has migrated the Git Credential Manager tool to
its own org:
https://github.com/GitCredentialManager
The team has also updated the name from Git Credential Manager Core to Git
Credential Manager. Updating links that currently point to the Microsoft
org and old name according to this change.
